Kurokawa Award (Case Report Category) Recipient’s Remarks

I am honored to receive the Kurokawa Award at the American College of Physicians Japan Chapter Annual Meeting 2023.

I presented the case report titled “Spontaneous bladder rupture with profound hyponatremia and pseudo-renal failure”. Spontaneous bladder rupture is uncommon and there are few case reports that lead to profound hyponatremia. We repeatedly discussed from the perspective of pathophysiology why severe hyponatremia occurred in this case. I realized that the learning points became clear when I imagined how I would teach residents.

This was my first time to present a case report in English. Dr. Kiyoshi Kinjo gave me a lot of guidance. I would like to take this opportunity to thank Dr. Kinjo and my colleagues who worked very hard with me on this difficult case.

As a doctor who works on a remote island, it is my important job to share interesting cases that are unique to remote islands. I will continue studying and working hard to grow my skills as a good clinical educator.

Finally, I would like to express my appreciation to everyone involved in the ACP Japan Annual Meeting 2023.

ACP日本支部年次総会・講演会2023　黒川賞（症例報告部門）を受賞して
沖縄県立宮古病院　砂川 惇司

この度はACP日本支部年次総会2023において名誉ある黒川賞を受賞することができ、大変光栄に思います。

「膀胱自然破裂に随伴した偽性腎不全、低ナトリウム血症」の症例を発表いたしました。非外傷性の膀胱破裂の頻度は少なく、その中でも重度の低ナトリウム血症に至った症例報告は限られています。なぜ低ナトリウム血症に至ったのか、病態生理から考察を深めました。今回の発表では、もし次に似たような症例に出会った場合に、どのように研修医に教えるかと想像することで学習ポイントが明確になったと感じました。

私は英語でのプレゼンテーションは初めてでしたが、金城紀与史先生から繰り返しご指導いただき無事に発表することができました。感謝申し上げます。また本症例の入院治療に一緒に携わった同僚の先生方にも感謝いたします。

私は現在沖縄の離島（宮古島）で勤務しており、今後は離島ならではの症例も報告したいと思っています。また指導医としても成長できるよう精進していきたいと思います。最後に今回発表する場を提供いただきましたACP日本支部の皆様に御礼申し上げます。ありがとうございました。

Kurokawa Award (Clinical Research Category) Recipient’s Remarks

I am honored to receive the Kurokawa Award at the ACP Japan Chapter Annual Meeting 2023.

At the meeting, I presented our study entitled “Loneliness Is Associated with Low-Health Interest: A Nationwide Cross-Sectional Study”. Our study used a Japanese nationwide questionnaire survey and showed that people who feel loneliness were less interested in their health. Recently, loneliness is reported to be associated with various diseases and mortality. Moreover, health interest is associated with disease prevention or treatment behaviors. We were not able to detect causality in our present study, however, we intend to conduct further research on the theme of loneliness and health.

I have worked as a nephrologist, and I have done a few clinical researches on chronic kidney diseases. While doing my researches, I noticed the importance of study design and epidemiology. I am now studying at a graduate school of public health. Receiving this award is encouraging my carrier and maintaining motivation. I am deeply gratified about this award. Thank you so much.

Research cannot be done by one single person, but requires the cooperation of many different specialists to make it better. I would like to express my gratefulness for enabling me to receive this award. I appreciate Dr. Takahiro Tabuchi of the Osaka International Cancer Center for providing the data, and Dr. Yoshiharu Fukuda of Teikyo University Graduate School of Public Health for his guidance, and all those who involved to our study. Finally, I would like to express my sincere gratitude to the American College of Physicians Japan Chapter for organizing and supporting this conference.

ACP日本支部年次総会・講演会2023　黒川賞（臨床研究部門）を受賞して
帝京大学大学院公衆衛生学研究科　西沢 蓉子

この度はACP日本支部2023年次総会において黒川賞をいただき，大変光栄に存じます。

今回、“Loneliness Is Associated with Low-Health Interest: A Nationwide Cross-Sectional Study”というタイトルで発表させていただきました。全国規模のアンケート調査を分析したところ，孤独を感じている人は健康への関心が低くなるという結果でした。孤独はさまざまな疾患と関連し，さらには死亡率とも関連することが分かってきています。また，健康への関心は疾病予防行動や受療行動と関連することが明らかになっています。今回の研究で孤独と健康への関心について因果関係にまで言及することは困難ですが，さらなる研究を行っていければと考えています。

私はこれまで腎臓内科医としていくつかの臨床研究を行ってきました。そのなかで研究デザインと疫学の重要性を認識し，公衆衛生大学院へ進みました。まだ至らない点も多いですが，今回黒川賞をいただいたことを励みに，今後も学び続けていきたいと思います。

研究というのは一人で行うことはできず，様々な人の協力が必要で，様々な人の目が入ることでより良いものになると実感しています。今回の研究もその例外ではありません。この場をお借りし，データを提供くださいました大阪国際がんセンターの田淵貴大先生，ならびにご指導くださった帝京大学大学院公衆衛生学研究科の福田吉治先生，ほかサポートいただいた関係者のみなさまに，深く御礼申し上げます。そして最後に，学会を運営し支えていただいている米国内科学会日本支部の関係者の皆様に，心より感謝の意を表したいと思います。
